My Plan

As a member, you have questions about your health plan coverage. Answers to many of these questions can be found in the documents listed in this section. Simply select your area and Plan from the drop-down menus to begin. Your Plan name can be found on the upper right corner on the front of your ID card.

Evidence of Coverage

The Evidence of Coverage is the handbook that explains how your plan works, your rights and responsibilities and includes benefit information. Refer to this document when you have a question about your coverage, how to disenroll or what to do if you have a complaint.
