States have programs that help pay your Medicare premiums and in some cases, copays and coinsurance. Physicians Health Choice has partnered with Social Service Coordinators® (SSC) to assist you in applying for these valuable benefits. Plus, you may also be eligible for help with costs of obtaining prescription drugs through a Medicare drug plan.
To apply for these money saving programs, or to find out if you qualify, call:

Your participation is voluntary and you are not obligated to provide any information. There is no cost to you to apply for these programs or for SSC’s help and your health plan benefits will not be affected. Any information you provide to SSC will be kept private and will not be shared with anyone outside of the application process. SSC will only use this information to see if you qualify for Medicaid, Extra Help or Golden Touch® programs. While SSC will assist with your application, only the Medicaid agency in your state can approve your application for benefits.

The Medicare Savings Program may save people with Medicare as much as $1,100 per year in Medicare Part B monthly premiums. This program may also pay:
The Low Income Subsidy Program (also known as Part D Extra Help) helps people save money on their prescription drug costs. Individuals who qualify pay $2.60 or less for each generic drug and $6.50 or less for each brand-name drug. This program may also help pay:
